(a) A health professional may receive a loan reimbursement
or stipend only if the health professional signs a contract agreeing
to provide health care services in a medically underserved community
for a period of at least 12 consecutive months.
(b) Loan reimbursement and stipend recipients must
comply with applicable state law, program requirements and grant terms,
including but not limited to the following:
(1) provide health care services in the qualifying
community for the duration of the obligated service period;
(2) provide health services to participants of government-funded
health benefits programs in the qualifying community;
(3) cooperate with the department in its efforts to
collect information and data relevant to the program;
(4) notify the department in writing within 30 days
of any change in address or other relevant contact information during
the contract term; and
(5) submit periodic statements to the department certifying
compliance with program and contractual requirements, in accordance
with reporting timelines provided in the contract. Failure to comply
with reporting requirements may result in the withholding of funds
and/or termination of the award.
(c) Funds will not be released to the recipient until
the recipient has begun working in the qualifying community. Failure
to satisfy all award terms will result in termination of the contract
and required repayment to the department.
