(a) A health care entity located in a county designated
as a medically underserved area and/or a health professional shortage
area is eligible to register to participate in the program.
(b) The department ranks eligible communities for program
participation based on a community's need for primary care physicians
and other factors including, but not limited to, the county's MUA
score or HPSA degree of shortage designation.
(c) Communities register for the program by completing
a registration form prescribed by the department during a time period
specified by the department.
(d) A community that is accepted into the program and
that is matched with an eligible physician is ineligible to re-register
for participation until the physician has fulfilled the service obligation.
