RULE §65.153 | Application for Permit |
An applicant for an AMP shall complete and submit an application on a form prescribed by the department. The application shall contain the description, including make, model, color, and registration number of each aircraft to be used. The name of each pilot shall be shown exactly as it appears on the pilot's FAA license, along with a current address and date of birth of the applicant (date of birth not applicable if corporation), Social Security number, and the name, address, hunting license number or driver's license number, and date of birth of each pilot. |
