(a) Caregivers must maintain a cumulative medication
record of all:
(1) Prescription medication dispensed to each child;
and
(2) Nonprescription medications and supplements that
are dispensed to a child under five years old.
(b) The cumulative medication record must be updated
within 24 hours of administering medication.
(c) Caregivers must maintain the medication record,
which must include:
(1) Child's full name;
(2) Prescribing health-care professional's name, if
applicable;
(3) Reason medication was prescribed, for prescription
medication;
(4) Medication name, strength, and dosage;
(5) Date (day, month, and year) and the time the medication
was administered;
(6) Name and signature of the person who administered
the medication;
(7) Child's refusal to accept medication, if applicable;
and
(8) Reasons for administering the medication, including
the specific symptoms, condition, and/or injuries of the child that
the caregiver is treating, only for:
(A) PRN psychotropic medication; and
(B) Nonprescription medications and supplements for
children under five years old.
(d) You must document in the medication record any
non-prescription medication or supplement that is given to the child
and how often the child receives the medication or supplement.
(e) Caregivers must document any prohibited prescription
medications (for example, medication allergies or contraindications)
or prohibited nonprescription medications and supplements in the medication
record.
(f) You must incorporate the medication record into
the child's record.
