| RULE §354.1091 | Additional Claim Information Requirements |
In addition to the general requirements in §29.1 of this title (relating to Claim Information Requirements), the following information is required on claims for laboratory and x-ray services and radiation therapy; the name, address, and appropriate identification number (if known) of ordering physician. |
