| (a) There is a Category D - Statewide Reporting Measure
Bundle for each provider type, as described in the Measure Bundle
Protocol.
(b) Each Category D - Statewide Reporting Measure Bundle
consists of one or more measures, as described in the Measure Bundle
Protocol.
(c) The valuation for each measure in a performer's
Category D - Statewide Reporting Measure Bundle for each DY is equal
to the valuation of the performer's Category D - Statewide Reporting
Measure Bundle for the DY divided by the number of measures in the
Category D - Statewide Reporting Measure Bundle, so that the valuations
of the measures are equal.
(d) A performer must report on a measure in the Category
D - Statewide Reporting Measure Bundle for its provider type as described
in the Measure Bundle Protocol for a DY no later than the second reporting
period of the DY to be eligible for payment of the measure for the
DY.
