| (a) The registrant shall amend the registration or any renewal not later than the 90th day after the date on which a change in the information provided in the registration or renewal occurs. (b) The amendment to the registration shall set forth the following information: (1) the name of the registrant; (2) the date of the last filed registration or renewal and any identification number assigned by the secretary to that registration or renewal; and (3) an identification of the information that has changed and the manner in which the information has changed. (c) The amendment shall be signed and sworn to by or on behalf of the registrant in the same manner as an original application for registration or any renewal thereof. (d) If the amendment is filed to reflect a change in the address of the health spa, the amendment must be accompanied by a rider to any security bond reflecting the change in address. |
