| (a) As part of the Health, Social, Educational, and
Genetic History report, you must compile the following information
for a child you are considering for adoption placement:

(b) In addition, you must document the following in
the child's record:

(c) This section does not apply to an adoption by the
child's:
(1) Grandparent;
(2) Aunt or uncle by birth, marriage, or prior adoption;
or
(3) Stepparent.
