CHAPTER 343SECURE JUVENILE PRE-ADJUDICATION DETENTION AND POST-ADJUDICATION CORRECTIONAL FACILITIES
SUBCHAPTER DSECURE POST-ADJUDICATION CORRECTIONAL FACILITY STANDARDS
RULE §343.612Admission Records

The facility shall obtain and record the following information at the time the resident is admitted into the facility:

  (1) date and time of admission;

  (2) name;

  (3) nicknames and aliases;

  (4) social security number;

  (5) last known address;

  (6) adjudicated offense;

  (7) name of attorney;

  (8) name, title, and signature of delivering individual;

  (9) sex;

  (10) race;

  (11) date of birth;

  (12) citizenship;

  (13) place of birth;

  (14) name, relationship, address, and phone number of the resident's parents, legal guardians, or custodians; and

  (15) primary language of the resident and the resident's parent, legal guardian, or custodian.
