| (a) The suicide prevention plan shall be reviewed in
consultation with a mental health provider at least once each year,
no later than the last day of the calendar month of the previous year's
review. Documentation of the review shall be maintained on the actual
plan or in the form of a dated and signed letter from the mental health
provider indicating the individual's title and the date that the review
occurred. The documentation shall include the mental health provider's
signature, title, and date of the review.
(b) The suicide prevention plan shall be disseminated
or made available to all facility staff having responsibilities named
or enumerated in the facility's suicide prevention plan.
