(a) For small schools holding a multiyear certificate,
the certificate of approval shall be renewed at least every three
years, or more frequently as determined by the Agency. A complete
application for renewal of a certificate of approval shall consist
of the following:
(1) complete and correct annual financial statements,
as specified in this chapter, demonstrating the school is financially
stable and capable of fulfilling its commitments for training;
(2) the renewal fee specified in this chapter; and
(3) any other revisions or evidence necessary to bring
the school's application for approval to a current and accurate status
as requested by the Agency.
(b) For all other schools, the certificate of approval
shall be renewed annually. A complete application for renewal of a
certificate of approval shall consist of the following:
(1) a completed application for renewal form provided
by the Agency;
(2) complete and correct annual financial statements
for the most recent fiscal year demonstrating the school is financially
stable and capable of fulfilling its commitments for training;
(3) the renewal fee specified by this chapter; and
(4) any other revisions or evidence necessary to bring
the school's application for approval to a current and accurate status
as requested by the Agency.
(c) The effective, expiration, and issuance dates are
indicated on the certificate of approval. The Agency may reflect the
date of renewal as the date following the date of expiration of the
prior certificate of approval, if the school submitted a timely request
for renewal and met all of the requirements contained in this chapter
for renewal.
(d) The complete renewal application shall be postmarked
on or before the due date as indicated in the Act.
