| (a) Petitions shall be in writing and shall:
(1) state the petitioner's interest in the subject
matter, the facts relied upon, and the relief sought; and
(2) cite the specific code provision(s) or other appropriate
law.
(b) The original of each petition, pleading, motion,
brief, or other document permitted or required to be filed with the
department in a contested case shall be signed by the party or the
party's authorized representative.
(c) All pleadings filed in a contested case shall be
printed or typed on 8-1/2 inch by 11 inch paper in no smaller than
11 point type with margins of at least one inch at the top, bottom,
and each side. Each page shall be numbered at the bottom. All text,
except block quotations and footnotes, shall be double spaced.
