(a) Each rail transit agency shall develop, and document
in its public transportation agency safety plan, a process to identify
and resolve hazards during its operation, including any hazards resulting
from a subsequent system extension, rehabilitation, or modification,
from operational changes, or from other changes within the rail transit
environment.
(b) The safety risk management process must, at a minimum:
(1) define the rail transit agency's approach to hazard
management and the implementation of an integrated system-wide hazard
resolution process;
(2) specify the mechanisms used for the on-going identification
of hazards;
(3) define the process used to evaluate identified
hazards and prioritize them for elimination or control;
(4) identify the mechanism used to track through resolution
the identified hazards;
(5) define minimum thresholds for the notification
and reporting of hazards to the department; and
(6) specify the process used by the rail transit agency
to provide on-going reporting of hazard resolution activities to the
department.
(c) A rail transit agency shall report to the department
hazards in accordance with the State Safety Oversight Program Standard.
