(a) All STAR Kids members have access to service coordination.
Service coordination includes:
(1) face-to-face and telephonic contacts between the
member or the LAR and the service coordinator;
(2) development and maintenance of a comprehensive,
person-centered individual service plan (ISP);
(3) coordination to assist the member in accessing
services provided by the STAR Kids MCO;
(4) coordination to assist the member or the LAR in
accessing services provided by other community entities or service
providers; and
(5) transition planning, beginning no later than age
15, to help the member prepare for changes in life circumstances and
changes in available healthcare services to ease the shift to adulthood.
(b) STAR Kids members with an identified need for more
intensive service coordination are assigned a single, named service
coordinator by the STAR Kids MCO. All STAR Kids members have access
to a single, named service coordinator upon request.
