RULE §215.27Complaints

(a) Complaints alleging violations of Occupations Code, Chapter 2301 or Transportation Code, Chapters 503 and 1000 - 1005 shall be in writing, addressed to the department, and signed by the complainant. Complaint forms will be supplied by the department for the purpose of filing complaints.

(b) A complaint shall contain the name and address of the complainant, the name and address of the party against whom the complaint is made, and a brief statement of the facts forming the basis of the complaint.

(c) If requested by the department, complaints shall be under oath. Before initiating an investigation or other proceeding to determine the merits of the complaint, the department may require from the complainant additional information necessary to evaluate the merits of the complaint.
