(a)A licensee or registrant shall notify the Board
in writing of any legal name change, or change of physical or
email address, not later than thirty (30) days after the change
becomes final. [After receiving notification of a change of name,
along with the required fee, the Board shall update its records and
issue a new pocket card reflecting the change.]
(b)A Plumbing Inspector shall notify the Board in
writing of each political subdivision the Plumbing Inspector is employed
by, or has contracted with, for the purposes of performing plumbing
inspections and any changes in contract or employment status within
thirty (30) days of status change. The written confirmation of contract
or employment must be provided by an authorized representative of
each political subdivision.
(c)A licensee or registrant shall notify the Board
in writing of any change to his or her primary place of employment
upon renewal of his or her license or registration.
[(d)A licensee or registrant shall
notify the Board of a change of mailing address as soon as practicable
and no more than thirty (30) days after the change becomes final.]
The agency certifies that legal counsel has reviewed
the proposal and found it to be within the state agency's legal authority
to adopt.
